Yes, any time you have an epic item (or item recipe) there is a chance of an epic gem (or gem recipe) to accompany it. I've never seen 1 single char get more than 1 item (epic or not; not including runes/potions) from the same fight. I have seen multiple items drop at the end of the fight, but only in a group and the items went to different chars.
